I appreciate all the useful replies, and I have provided a summary as shown below. For the first part of my question, the standard I was looking was CCMA (Closure & Container Manufacturers Association), I just did not remember the name. This applies to both blow molded bottles and injection-molded caps.
The second part of my question was what are the options available for creating internal threads for a plastic injection molded cap, and how does the cost relate to each method. Glad some of you where able to read my mind as I probably did not state my question as clearly as it should have been.
