I have used Jeff Carlson quite a bit for training here at my current company, as he's especially convenient for us being in the Denver/Boulder area. I have had good feedback from my users that have taken his intro class, although I myself have never taken one from him. I usually ask my users for feedback after a class so I can determine if an instructor is worth bringing back, and with the good reviews Jeff gets, he is actually my go-to instructor for intro classes. He also did a basic surfacing class for us a while back and that toorecevied good reviews.
We have also had Bart from Design Engine out for some higher-level surfacing/ISDX classes. My guys that are heavy into surfacing were able to pick up some really good techniques from him. As someone else said, he doesn't just teach you mouse clicks, he teaches techniques that can be applied in any CAD package. I think this is what you need for higher-level surfacing gurus, because they usually already know where all the mouse clicks are, they just need some guidance on making more robust surfaces.
