I know that using the Med Device Template, if you have a DHF or DMR linked to an End Item, an option appears in the Actions menu for the End Item saying "View DHF" or "View DMR". Clicking this link brings up a new window that shows the DHF structure in its own window along with the product structure. That is one place to start... see how they generate that report.
The other place you might look is in the product structure reports. When you are viewing a product structure, there is a "Related Reports" dropdown that you can use to see things like Multi-Level Where Used, Single Level BOM, etc. If you can see how those reports are being generated, you might be able to apply the same logic to a custom query and use that to get your information.
Finally, note that using the Query Builder, one of your output format options is PDF. If you build it in the Query Builder, you can output your PDF.
